Fit (sick) notes

If you are unable to work due to illness, you may need to provide your employer with evidence of your sickness. This guide explains when you need a fit note and how to request one from our clinic.

If you have been off work for 7 days or less, you do not need a fit (sick) note

Instead, you can self-certify your illness by filling out an SC2 form for your employer. Click here for more information

If you are sick and unable to work for more than 7 consecutive days, your employer will usually ask for medical evidence. This is where a formal fit note is required.

How to Request a Fit Note

Please Note: We cannot issue a fit note before you have been absent from work for 7 days, and we cannot pre-date a fit note for a future illness.

If this is the first time you are requesting a fit (sick) note because of this problem, or this is a new period of absence: You will need to submit a new online request to arrange an appointment with a clinician.

If you have already discussed this with a clinician and you are requesting a fit note extension: Please complete the below form. The GP may ask that you arrange a review to discuss your absence.

Once your fit note is processed, it will be sent to you digitally via text or email so that you can easily forward it to your employer. If you require a printed paper copy of your fit (sick) note, please clearly specify this and we will arrange for you to collect this from reception.
