Is it really anonymous?

Yes. Your feedback is completely confidential. It will never be linked to your medical records, and the staff providing your care will not know which response was yours.

Do I have to fill it out?

No. The test is completely voluntary, but we highly encourage you to take part. Your feedback is incredibly valuable to us.

Can a carer or family member fill it out for me?

Yes. If you are unable to fill out the form yourself, or if you are a parent or carer of a patient, you can complete the survey on their behalf.
