Register with our practice

Hugglescote Surgery welcomes new patients from Hugglescote and some surrounding areas.

In line with current NHS England regulations, everyone living in England is entitled to register with a GP surgery and receive free primary care. You do not need to prove your immigration status, provide a permanent address, or have an existing NHS number to join our practice.

While documentation is not a legal requirement to receive healthcare, providing supporting information is highly beneficial. It helps us accurately locate your central NHS records, prevents duplicate medical files, and confirms that you live within our designated practice boundary. Alternatively, new patients will need to complete a New Patient Registration Form which can be collected from Reception. Please read all details in this form carefully as it will provide us with useful information whilst we wait for your medical records to arrive from your previous doctor.

Medical treatment is available from the date of registration.

To help us process your child’s registration smoothly, it is highly beneficial if you can provide the following:

  • The child’s NHS number (found in the Red Book)
  • The child’s Birth Certificate
  • Documentation confirming parental responsibility or legal guardianship if required
  • Completed form called New Patient Registration Form – Child Under 16

Registering New Babies

If you have just had a baby, congratulations! All you need to do to register them with our surgery is bring their Red Book in and our Reception team will do the rest for you.

Completing Registration as a Patient from Overseas

If you have recently arrived in this country, you will need to bring the following to Reception:

  • Passport or Visa
  • Bank Statement
  • Completed New Patient Registration Form – Adult 16+ or Child Under 16

Catchment Area (Practice Boundary)

Named GP

Hugglescote Surgery is required by the government to allocate all patients a named accountable GP (identified as your usual GP on your medical records) who is responsible for overseeing your care.

The Named Accountable GP’s at the Practice are Dr T. Gauhar, Dr D. Greenyer, Dr H. Noble and Dr M.Patel

Whilst we encourage all of our patients to see their own GP to support continuity in the care being provided, patients are entitled to choose to see any GP or nurse in the Practice.

Due to operational reasons you may not see your own GP when requesting an appointment, however, we will always endeavour to give you an appointment with your own GP where appropriate and availability allows.
