Suitable Forms of ID

You do not need proof of address or immigration status, ID or an NHS number to register with a GP. However, if you are able to provide the following, it will help us:

• A form of Photographic ID
  • passport
  • photocard driving license
• Proof of your address
  • a recent utility bill
  • payslip
  • benefits agency letter
  • papers from the home office
