To help us offer the appropriate advice, please print and complete the 'Travel Form' at the bottom of this page and hand it in to the Surgery. You can also collect a travel form from reception. This form requires information on which countries and areas within countries that you are visiting in order to determine what vaccinations are required. There is further information about countries and vaccinations required on the links below.
Once you have handed in your completed form, the Nurses will then assess it and if you require any vaccinations or advise relating to foreign travel you will be contacted to arrange an appointment with the practice nurse.
It is important to attend your appointment as early as possible - at least 8 weeks before you travel. This allows the vaccines an appropriate period of time to work and also allows us time to order in the vaccines, if required.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
