New Patients

Hugglescote Surgery welcomes new patients from Hugglescote and some surrounding areas. To check whether you live within our practice boundary, please call Reception on 01530 832109, or check the Practice Boundary Map.

How to Register

New Patients will need to complete a New Patient Registration Form which can be collected from Reception, or can be downloaded here and printed at home. Please read all details in this form carefully as it will provide us with useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the Healthcare Team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration.

If you would like to register for online access at the same time, please also complete the form provided on the Online Services page.

Completing Registration for Adults

To complete your registration, you will need to bring the following in to Reception:

• Proof of your address (check suitable forms of ID)
• One form of photographic ID (check suitable forms of ID)
• Completed New Patient Registration Form

Completing Registration for Children

To complete registration for a child, you will need to bring the following to Reception:

• the child’s NHS number (found in the Red Book)
• the child’s Birth Certificate
• the child’s Completed New Patient Registration Form

Completing Registration as a Patient from Overseas

If you have recently arrived in this country, you will need to bring the following to Reception:

• Passport or Visa
• Bank Statement
• Completed New Patient Registration Form

Registering New Babies

If you have just had a baby, congratulations! All you need to do to register them with our surgery is bring their Red Book in and our Reception team will do the rest for you.
