MEDICINE WASTE CAMPAIGN 2019 AND HOW IT WILL AFFECT YOU

Medicines over-ordering and drug wastage has been identified as a serious and growing problem within the NHS.  It is estimated that as much as three hundred million is wasted every year on unused or partially used medication.  We are asking all our patients to look at the medications they order regularly and ask that they only order the medications that are needed.  In an attempt to reduce the amount of waste prescription medications and inline with other Leicestershire practices and other areas in the UK, we will no longer be accepting third party requests for medication.  This means that all repeat requests must be ordered by the patient themselves as only they know exactly what they require.

The date we have nominated for our new process to start is: Tuesday 1st January 2019

EXPRESS CONSENT TO SHARE YOUR MEDICAL RECORDS

Is it ok if we share important health information about you with other health professionals,  if they need it to look after you?

Most of our patients already have a summary care record, where medications and allergies can be viewed, if you choose to add additional information to this such as relevant problems,   communication needs etc, this will further increase the quality of your care - Quicker access, diagnosis and treatment.

If the answer is yes - please contact the reception who can update your medical record.

New Patient Registration

If you live within our practice boundary you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.

Once you have confirmed you are within the practice boundary, you will need to complete a registration form which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You can collect both of these forms from reception, or use the link below to print of your own form.

Please return all forms together along with your ID to reception. Incomplete registration packs will not be taken in.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration.  Please contact reception for further information.
